TRANSPORT RELEASE STATEMENT:

I/We permit my/our child, to be transported to and from Legana Youth events and activities by approved and screened adult leaders or hired professional transport providers between January 1, 2026 and December 31, 2026, unless I/we notify Legana Youth in writing.

Parents/Guardians will be notified of ALL & ANY transportation in advance including the destination and the drivers. All drivers will ALWAYS be Vaild Full Open Licenced drivers with a minimum of two approved leaders in each vehicle.

I/We understand that transportation may be provided by private vehicle, hired van/bus, or public transport and that only adult drivers with Valid Full Open Licenses, who meet Legana Christian Church’s safety requirements, will be permitted to transport youth. I/We release Legana Christian Church and its representatives from liability for transportation-related incidents except in cases of proven negligence.

GENERAL RELEASE STATEMENT:

1. General Participation Release

I/We, the parent(s)/guardian(s) hereby grant permission for my/our child to participate in Legana Youth activities facilitated by Legana Christian Church from January 1, 2026 to December 31, 2026, unless revoked in writing.

I/We acknowledge that all events will be supervised by adult leaders and that caregivers will be notified in advance of dates and destinations of any offsite activities. I/We understand that reasonable measures will be taken to ensure the safety and well-being of all participants.

2. Behaviour and Discipline Acknowledgement

I/We understand that participation in Legana Youth activities requires respectful and responsible behaviour from all participants. I/We agree that if, in the opinion of the adult leaders, my/our child behaves in a manner deemed disruptive, unsafe, or inconsistent with the values of Legana Youth, they may be asked to leave or be sent home at our expense.

I/We accept full responsibility for arranging or covering the cost of any early departure required due to misconduct or health-related concerns.

3. General Liability and Risk Waiver

I/We release Legana Christian Church, its staff, volunteers, and representatives from any liability for accidental injury, illness, or unforeseen incidents occurring during any youth activity, including while travelling to and from such events, to the extent permitted by law.

I/We understand that while every effort will be made to provide safe and secure environments, participation in youth activities may involve inherent risks. I/We assume these risks voluntarily on behalf of our child.
